The above information is supplied solely for the purposes of credit determination and is warranted to be accurate and true. In order to supplement the information contained herein, I/We authorize Elma Steel and Equipment Ltd. to conduct an investigation as may be considered necessary and I/We authorize any reporting agency to supply such information as required. In addition, I consent that Elma Steel may have collected personal information as defined by the Personal Information Protection and Electronic Documents Act (PIPEDA) and other provincial legislation and hereby consent to the use of this information for the purposes as described above. I/We agree to make payment in accordance with terms specified on an Elma Steel and Equipment Ltd. Invoice. Terms are Net 30 unless otherwise stated or arranged. I/We also understand that Elma Steel and Equipment Ltd. reserves the right to reclaim its property if payment is not made as promised. Interest at the rate of 24% per annum (2% per month) is applicable to all overdue accounts.
